Các tia vây lưng cứng (tổng cộng): 11; Các vây lưng mềm (tổng cộng): 9; Tia cứng vây hậu môn 3; Tia mềm vây hậu môn: 10 - 11. The genus Grammatonotus surugaensis differs from Callanthias in having 1 spine on the operculum, 13 branched caudal fin rays, and fewer scales in longitudinal series. Several caudal fin rays produced into filaments. Body orange-red; tip of spinous dorsal fin yellow. Attains 8 cm SL.
