Épines dorsales (Total): 9; Rayons mous dorsaux (Total): 11; Épines anales 3; Rayons mous anaux: 10; Vertèbres: 25. Head with six blue bands with three slightly oblique, extending downward from lower part of preopercle onto inter opercle. The top of the head and back brownish; lower two-thirds of the head and body whitish. There is a brilliant blue mid ventral strap on throat. With five to six predorsal scales (rarely 4); two cheek scale rows behind eye, three below eye. A low, scaly sheath at the bases of dorsal and anal fins. Scales large.
Inhabits coral reef and rock reef (Ref. 75154).
